The NHS is undergoing a transformation to meet the needs of an ageing population. With an increasing number of older adults living with multiple long-term conditions and frailty, the system is focused on integrated, person-centred care that prioritises prevention, early intervention, and effective care coordination. The aim is to ensure older people receive the right care at the right time, reducing hospital admissions and supporting their independence.

This award recognises NHS-led projects that have transformed care delivery for older people. Judges are looking for projects that drive measurable improvements in access, patient experience, and health outcomes. Successful projects will demonstrate how they have improved care coordination, streamlined service delivery, and achieved sustainable impact within the NHS.

Eligibility

All NHS organisations (including providers, partnerships and systems), General Practice and primary care organisations. 

Ambition

The challenge and context within which your project, person or organisation is set alongside your goals and targets whether quantitative or qualitative, and how this aligns with national priorities.

  • Describe the healthcare challenge faced by older people that your project aimed to address, and how it sought to improve care delivery through integration and coordination across services
  • What specific goals and outcomes were set for the project, and how were they determined?
  • What made this approach new or distinct from existing best practice?

Collaboration

The stakeholders' involvement in co-designing and delivering the project. How have patients, staff at all levels, communities and other parties worked together to realise the outcomes?

  • How were older people, families and carers involved in the design and implementation of the project, and how did their feedback shape the approach?
  • Provide evidence of how staff across different teams, organisations and system partners contributed to co-designing and delivering the work
  • How did cross-organisational working enable results that would not have been possible otherwise?

Impact

The measurable benefits delivered to patients, staff, your organisation or the wider system. Provide data and evidence showing improvements to outcomes, quality, access, equity or efficiency.

  • Provide evidence of measurable improvements in patient outcomes, such as reduced hospital admissions, improved care coordination or enhanced patient experience. Quantitative evidence is essential but qualitative feedback from patients, carers and staff is also welcomed
  • Highlight any innovations introduced that improved the delivery of care for older people
  • Demonstrate any financial savings, operational efficiencies or improved use of resources resulting from the project

Scale

How your work has been shared, adopted or replicated beyond your immediate team or organisation. This includes dissemination through publications, presentations, toolkits, partnerships or inspiring similar initiatives elsewhere.

  • How has the project been scaled or replicated within other departments or NHS trusts?
  • What evidence supports the replicability of the project across the NHS or other settings?
  • How have the project's results and learning been shared within the NHS and beyond?

Sustainability

The potential for the project/work to continue and create lasting impact. Evidence of how it can be sustained or built upon.

  • Demonstrate how the project's improvements are sustainable and will continue to benefit older people in the long term
  • How are the changes being embedded into business as usual, and what resource or funding is in place to sustain them?
  • What steps are being taken to ensure continued improvement in older people's care as needs and pressures evolve?
