Each guest will be asked to provide their full name on arrival to gain entry to the awards. It is therefore imperative that your guest list is up to date.

  • Refer to the original email from the awards support team for the number of places you have booked.
  • Names that don't reach us by the deadline could lead to refused or delayed entry and missed dietary requests.
  • These security team will be undertaking bag searches on arrival, we ask for your full support and cooperation in this matter. 

Completing the guest list form. 

  1. Each guest must have a unique email address - you cannot use the same email address for more than one attendee: trying to do so will only save one person on the system.
  2. Please ensure that you enter your Unique reference number for each form you submit. You will find your reference number in the email from the HSJ Awards Team. The number beginning with OP-******
  3. Please keep your own record of your award attendees - this form is a means to record guest attendance; it cannot be used to edit or view previously submitted attendees, or manage your booking information.
  4. If you do not have all the details/names for your attendees you can add later, using this form. Please keep your own records so that you know who has been submitted so far.
