As the front door to the NHS, consistent delivery across PCNs and primary care are the backbone to the health and care system. Faced with financial constraints and the enormous demand, primary care has continued to deliver world class care in difficult circumstances. Establishing digital and hybrid access to services, adopting new ways of working, collaborating with place-based and system partners in the delivery of care are just some of the challenges organisations continue to embrace. 

The winner of this award will be an organisation which is facing these challenges head on, with a focus on outcomes and staff wellbeing. Judges will be looking for initiatives which deliver performance against targets, clinical quality and safety. A successful candidate will need to demonstrate initiatives which drive inclusivity and representation across the organisation and investment in widening participation. Judges will want to see clear evidence of sustainable finances and delivery of value for money.  

The award will recognise those providers which are offering excellent, patient-centric care built on strong engagement between clinicians within and beyond the organisation. We are particularly seeking organisations with a clear understanding of how they will need to adapt to create a viable long-term health economy. 

Eligibility

This award is open all public sector primary care providers in the UK. This includes NHS trusts, charities, GPs and federations etc.

Ambition

The challenge and context within which your project, person or organisation is set alongside your goals and targets whether quantitative or qualitative, and how this aligns with national priorities.

  • What are the biggest challenges your organisation has faced in recent years, and what is your vision for addressing them?
  • What goals and targets have been set, and how do these align with national priorities around access, workforce and financial sustainability?
  • Describe your focus on safety, clinical excellence and the cultural challenges you are working to address, including diversity and staff wellbeing

Collaboration

The stakeholders' involvement in co-designing and delivering the project. How have patients, staff at all levels, communities and other parties worked together to realise the outcomes?

  • How does the provider engage with its staff and involve them in shaping the vision for the organisation? Describe initiatives which focus on culture, participation and representation
  • How does the provider engage with patients and how are they involved in shaping services and care?
  • Describe initiatives with other organisations providing health and care, including integrated services, partnerships and local government, and how these contribute to care excellence

Impact

The measurable benefits delivered to patients, staff, your organisation or the wider system. Provide data and evidence showing improvements to outcomes, quality, access, equity or efficiency.

  • Briefly describe a single initiative in your supporting material that best represents the provider's vision in action and the difference it has made
  • Describe performance against targets, value for money management and the impact on patient outcomes, including testimonial evidence of initiatives which drive representation and inclusion
  • How does the provider improve patient safety, and how does a positive learning culture contribute to safety?

Scale

How your work has been shared, adopted or replicated beyond your immediate team or organisation. This includes dissemination through publications, presentations, toolkits, partnerships or inspiring similar initiatives elsewhere.

  • How does the provider work with other health & care and non-health organisations to spread best practice learning?
  • Describe a specific initiative with supporting evidence that demonstrates the provider's efforts to share improvements and what has been achieved as a result
  • What evidence is there that this approach could deliver positive outcomes in other settings?

Sustainability

The potential for the project/work to continue and create lasting impact. Evidence of how it can be sustained or built upon.

  • How does the provider demonstrate financial resilience and a viable long-term vision for its role within the local health economy?
  • How are improvements in care, culture and ways of working being embedded into business as usual rather than remaining one-off initiatives?
  • What evidence is there that the provider's model can be built upon or adapted as primary care continues to evolve?
