Connecting Services and Information Award
With added focus in the Long Term Plan there is little doubt that the only way to achieve true integration of care will be to ensure a steady flow of data and information between different parts of the system. Yet challenges remain: concerns over consent and the need to ensure interoperability chief among them.
The award is about recognising NHS initiatives where data sharing has made a real difference. Judges are looking for successful implementation of systems and technologies which have enabled improvements to patient care – improving outcomes, experience and supporting patients to look after themselves better while at the same time delivering efficiencies for staff.
This category is open to all parts of the health and social care sector including collaboratives, integrated care systems. The award is also open to solution providers helping the NHS and health sector organisations with integrated patient and resource data and information system.
- Describe the scope of information sharing including detail on resources and capacity as well as patient data.
- Outline the how sharing of information was managed prior to any improvements. How were targets identified?
- Who was involved in the work? Outline any partnership or collaborative structures.
- What targets were set to enable better patient care and improved allocation of resources?
- How have the initiatives employed in the sharing of data and information led to improved patient care, improved experience, improved outcomes and/or improved value for money.
- Judges are looking specific quantitative evidence covering patient data, capacity and resource management.
- Qualitative feedback from stakeholders across care systems and collaborations should be included in supporting information.
- Describe the financial impact of implementation, including evidence relating outcomes to value for money.
- Describe how the learnings from data and information sharing initiatives have been shared.
- How has best practice been implemented in other organisations and localities?
- How has the sharing of information through more efficient technologies, systems and processes been of benefit to patient experience and staff ability to deliver improved service?
- Provide qualitative and quantitative evidence that efficiencies have been achieved and experience has improved.
- Describe with evidence the engagement of staff in the development and implementation of information sharing initiatives.
- How have patients been engaged in the planning and implementation of improvements and what feedback has been sought since?
